Kidney Disease Education
Coordinated Kidney Care is proactive in offering Kidney Disease Education to our patients.  Stage 3 Kidney Disease (eGFR 30-59) and Stage 4 Kidney Disease (eGFR 15-29) is a critical time in the life of a patient with chronic renal failure.  If they have progressive disease, patients will need to understand what option are available to them as time goes on.  This could include the possibility of transplant, home dialysis, in-center hemodialysis or conservative care.  Furthermore, issues of diet, anemia and bone disease take on increased importance.

Our practice uses the three-part Kidney Sunrise program that explains all aspects of chronic renal failure to our patients.  This enables them to make informed decisions regarding their current and future care, to help them manage their disease with physical and mental health benefits. With many years experience caring for patients with chronic renal failure and patients on dialysis, Stephenie Swisher, CRNP and Kenneth A. Hile, CRNP lead this educational program.
